Case management is committed to helping individuals and families stay together by obtaining resources needed to grow and become active participants in their communities. Service recipients are adults with intellectual and developmental disabilities. Services include: identifying individual and/or family needs, completing a skills assessment in several life domains, coordinating and advocating for needed services, and monitoring the progress and effectiveness of those services.
