Community Case Management Services
Community Case Management Services provides case management support for children and their families, and adults with Mental Retardation and Autism. Services provided include: identifying individual and/or family needs, completing an assessment of skill in several life domains, coordinating and advocating for needed services, and monitoring the progress and effectiveness of those services. Case management is a commitment to helping individuals and families obtain the resources needed to grow and become active participants in their communities.
