Community Case Management Services
This Program provides case management support for children and their families, and for adults with mental retardation and autism. Services include: identifying individual and /or family needs, completing a skills assessment in several life domains, coordinating and advocating for needed services, and monitoring the progress and effectiveness of those services. Case management is committed to helping individuals and families stay together by obtaining resources needed to grow and become active participants in their communities.
